Community Paramedic (Currently Hiring in Madison, WI)

Community Paramedics will assist Inbound Health in supporting Hospital-at-Home patients by collaborating with Inbound Health healthcare partners to provide proactive care in the home while reducing the rehospitalization of Hospital-at-Home patients.  He/she will work collaboratively to provide patient-centered care focused on initiating activities to safely monitor and treat patients in support of individualized care plans.  The Community Paramedic will serve as a patient advocate and resource, helping patients identify and overcome barriers that may prevent successful compliance with their treatment plans. 

Responsibilities of Community Paramedics include:

– Establishes relationships with and provides patient home assessments and care to ensure patient safety and success in the program.

– Assesses, obtains history and treats patient in the home following department protocols, procedures and processes.

– Ability to work self-directed applying knowledge and critical thinking decisions for patients well being and solve complex acute patient care issues.

– Follows orders given for patient care to include medication administration and IV fluid resuscitation per Provider both verbal and written.

– Competence in procedural skills including, but not limited to, phlebotomy, vascular access device management, hydration therapy, medication administration (oral, intravenous, intramuscular), wound management, specimen collection.

– Provides education for biometric set up in patients home to patient and family.

– Collaborate with the patient, family, interdisciplinary team to provide effective care coordination.

– Share pertinent assessment findings with effective communication skills to collaborate with the remote provider and multidisciplinary team members.

– Ensure all interventions in the patient’s care plan are implemented with a patient centered approach.

– Maintain a therapeutic and professional patient provider relationship.

– Maintains daily paperwork and procedures.

– Performs documentation in EMR per policy and procedure.

– Identifies opportunities for improvements of services provided.

– Participates in performance improvement initiatives.

– Participates in education to improve quality of program.

– Operates and maintains equipment in accordance with manufacturer and company policy.

– Adherence to policy and protocol.

– Other duties as assigned.

– 2-5 years of experience as a paramedic in a 911 setting

– High School Diploma, GED or Vocational degree, AAS Paramedic degree preferred

– EMSRB Community Paramedic required

– Certified Paramedic – WI Emergency Medical Services Regulatory Board required (state specific)

– Nationally Registered Paramedic preferred

– ACLS-BLS Tier 3 – Multisource required American Heart Association

– PALS preferred

– Community Paramedic Certificate (state specific)

– Driver’s License required (valid / state specific)

– Strong clinical skills with assessment and evaluation of patient care.

– Ability to work independently in patients home, be organized and resourceful, with strong communication and collaboration skills.

– Knowledge of emergency medical care techniques and procedures

– Knowledge of community paramedicine concepts;

– Ability to thoroughly evaluate, assess, and differentially diagnose patient’s conditions and communicate clearly with professional medical personnel;

– Ability to follow oral and written instructions; ability to communicate effectively both orally and in writing;

– Ability to establish and maintain cooperative working relationships with medical providers, fellow employees, patients, and the public;

– Ability to prepare reports.

Shifts:  10-hour shifts
Hours: 10:00 a.m. – 8:30 p.m.
Weekend and Holiday rotation
Location:  Madison, WI
