Valley Health System and Inbound Health Partner to Launch Hospital at Home Program, Expanding Access for Patients

PARAMUS, NJ, June 23, 2025 – Valley Health System, in partnership with Inbound Health, a leading enabler of advanced care at home, is proud to announce a Hospital at Home program, set to launch this summer. This innovative program will deliver high-acuity inpatient care in the comfort and safety of patients’ homes – improving patient experience while maintaining the same standards of quality and safety found at The Valley Hospital.

Hospital at Home programs have gained national momentum as a proven, cost-effective model for care transformation – improving patient outcomes, reducing re-admissions, and helping health systems manage capacity and staffing more efficiently. As the United States population ages and the burden of chronic conditions rises, demand for flexible, home-based models has increased. In communities like Bergen County, New Jersey, where Medicare and MedSurg populations are growing faster than the national average, these models offer an especially valuable alternative for patients and providers alike.

Valley’s Hospital at Home program provides eligible patients with the same high-quality care they would receive at The Valley Hospital, in the comfort of their own home. Eligible patients must visit The Valley Hospital, where their care team will assess their medical condition and home safety protocols. If deemed eligible for the program, the patient’s care team will make a referral to Valley’s Hospital at Home program.

Patients in the Hospital at Home program will receive daily clinical assessments, 24/7 monitoring, medical supplies, and home visits from nurses and clinicians for common inpatient diagnoses, such as pneumonia, congestive heart failure, or cellulitis. The overall goal of the program is to help patients heal in a comfortable, familiar environment.

Key performance indicators for program success will include patient satisfaction, readmission rates, hospital-acquired infection (HAI) reduction, operational and financial metrics, and measures of patient preference and empowerment.
