Clinical team completes chart review and assesses patient to confirm program eligibility

In-hospital liaisons coordinate with patient and local hospital provider

Local hospital provider refers patient to the program

Nurse creates patient’s care plan and orders, and then coordinates with regional care team

Community paramedic or nurse visits patient’s home to set up equipment and perform home check

Community paramedic provides instruction and medications and coordinates with virtual provider

Nurse or community paramedic visits patient (based on clinical need) to deliver in-person care

Virtual hospitalists oversee care

Care team visits patient for needed imaging, infusion, labs, and therapy services
