    MADISON, WI, NOVEMBER 11, 2024 – SSM Health and Inbound Health, a leading provider of in-home, advanced care programs for health systems, announce another innovative new partnership to bring personalized, in-home, hospital quality health care services to patients. 

    This week patients at SSM Health St. Mary’s Hospital – Madison who meet specific criteria will have the option to be transferred home to receive services traditionally offered in the hospital with the launch of a new SSM Health at Home program – Hospital Care at Home.

    “We’re excited to offer our patients more choices for their care,” said Dr. James Lehman, SSM Health Wisconsin Regional Clinical Program Medical Director. “We recognize that many patients prefer to receive care in their own homes and research shows that they can recover faster there which leads to lower rates of hospital readmissions.”

    Patients eligible for the new program will benefit from personalized, daily in-home, and virtual visits by a dedicated team of specially trained physicians, nurses, therapists and other clinicians to ensure that they receive the highest standards of care. Each patient will receive a digital tablet to be used during the program to provide enhanced remote monitoring and round-the-clock assistance from the care team at the touch of a button.

    A specialized care coordinator will ensure seamless communication among the patient’s primary care provider, specialty caregivers and support network. Patients in the program will receive the same level of services and equipment typically offered in a hospital setting but within the comfort of their own home.

This investment is led by HealthQuest Capital with participation from existing investors Flare Capital Partners and McKesson Ventures.

    Inbound Health’s Series B financing arrives at a time when health systems are increasingly leveraging the home as a site of care.  The programs that Inbound Health powers are demonstrating key clinical, operational and financial benefits for its health system partners.  Importantly, these advanced care at-home models are reducing length-of-stay, decreasing readmission rates, and lowering avoidable complication rates (e.g., infection, delirium), all of which lead to a reduction in total cost of care.  Inbound Health is aligning economics with its health system partners through a value-based care approach.

    MADISON, WI, APRIL 23, 2024 – SSM Health and Inbound Health, a leading provider of in-home, advanced care programs for health systems, are excited to announce an innovative new partnership to bring personalized, in-home, post-hospital health care services to patients.  

    The first phase of this collaboration begins at SSM Health St. Mary’s Hospital – Madison this week with the launch of a new SSM Health at Home program – Recovery Care at Home. Hospitalized patients who meet certain criteria will have the option to be discharged home to receive services traditionally offered in a skilled nursing facility within their own homes. 

    “Research shows that patients prefer to receive in-home care and often recover faster within a familiar environment and are less likely to return to a hospital,”

    “Patients being cared for at home spend less time in bed and are able to be more mobile so they can continue in their daily lives with their families, friends and pets to achieve better health outcomes.” 

    Patients eligible for the new program will receive personalized, daily in-home and virtual visits from a team of specially trained physicians, nurses, therapists and other clinicians to ensure that they receive the highest standards of care. Each patient will receive a digital tablet to be used during the program to provide enhanced remote monitoring and round-the-clock assistance from the care team at the push of a button.  

    A care coordinator arranges for all of the patient’s care visits to ensure seamless communication among the patient’s primary care provider, specialty caregivers and support network. The program brings into the patient’s home the same equipment and services typically offered in a hospital or nursing home setting, including: home infusion services, laboratory and imaging services and durable medical equipment.

    MINNEAPOLIS, FEBRUARY 21, 2024 – Inbound Health, a nationwide provider of advanced care-at-home solutions for health systems and health plans, announces the release of Inbound InHome™, a proprietary patient management and analytics platform designed specifically for in-home advanced care. Inbound InHome was designed by Inbound Health clinicians and healthcare operators to deliver a technology-enabled, facility-level care experience in the comfort of the patient’s home. “When Inbound Health first started seeing patients in the home in May of 2020, we were leveraging a traditional biometric monitoring platform as our technology backbone,” said Dave Kerwar, CEO of Inbound Health. “While that allowed health system partners to remotely track patient vital signs, it lacked some major components required to conduct seamless, efficient, and highly coordinated care at scale.”
